Welcome and thank you for taking time to think about how you can improve your health and the health of our community! We know that everyone wants to live in a healthy hometown. But what actions are you willing to take to be healthier? How can your actions help others? This survey is designed to provide a few ideas for action, though we also are looking for additional ideas. Each question offers “other” as an option. Your new and creative ideas are welcome. The purpose of this survey is threefold: 1. To help you think about actions you can take to improve our community’s health. 2. To help community leaders focus resources. 3. To share new ideas about improving health.
The Community Health Priorities included in this survey were chosen by more than 100 leaders from Sedgwick County. A copy of the plan can be downloaded here. This plan calls for action that will lead to positive change in four areas: - Access to Healthcare - Obesity and Diabetes - Mental Health - Oral Health
In each priority area, reducing health disparities is a goal. Health disparities describe differences in health among groups of people that are avoidable and unjust.
This survey will take 5-10 minutes; as you complete it, be prepared to ask yourself how you can make a difference as an individual, as well as within organizations, at work, in your community, or within your faith community.
At the end, you can print a copy of your answers as a reminder about your options for action. Thanks in advance for participating. All responses will remain confidential and secure.
